Facial fat grafting has become increasingly popular for areas such as:
- Forehead
- Temples
- Under-eyes
- Nasolabial folds
- Lips
Fat grafting involves transferring fat harvested through liposuction into another area of the body.
Many people view it as:
“Natural because it uses your own fat”
“A safe way to restore volume”
And in some cases, it can indeed produce beautiful results.
However, at AVAN TOKYO, we believe:
“Facial fat grafting should never be recommended casually.”
The biggest risk: nodules and fat necrosis

One of the most important risks of facial fat grafting is:
“nodules” (fat necrosis / fibrosis)
After injection, fat cells must establish a new blood supply in order to survive.
If they do not,
- fat necrosis
- inflammation
- fibrosis
can occur.
This may lead to:
firm lumps or irregularities.
The under-eye area is especially delicate
At AVAN TOKYO, we are extremely cautious with:
under-eye fat grafting.
In fact, unless a patient has a very strong desire for it,
we often advise against the procedure.
Why?
Because the under-eye area has:
- extremely thin skin
- limited blood supply
- very little margin for irregularities
- difficult revision potential
Even with meticulous technique,
the risk of nodules can never be reduced to zero.
And unlike fillers,
fat cannot simply be “dissolved.”
In some cases, revision may require:
- surgical excision
- aspiration
- direct fat removal
which can be highly challenging.
“Natural” does not always mean “safe”
Recently, many patients assume:
“Because it’s my own fat, it must be safe.”
However, in facial fat grafting,
selecting the right area is critical.

“Knowing where NOT to inject”
is just as important as knowing where to inject.
Areas that are generally safer
Areas such as:
- Forehead
- Temples
are often more forgiving because they have:

- thicker skin
- better blood supply
- less visible irregularities

As a result,
nodules are less noticeable and fat survival is often better.
Skin thickness and blood flow matter
In facial fat grafting,
skin quality and vascularity
are extremely important.
Areas such as:
- Under-eyes
- Tear troughs
- Lips
require particularly careful consideration because they are:
- thin-skinned
- highly mobile
- vascularly delicate
In many cases,
avoiding injection may actually be the better decision.
Facial fat grafting is a highly delicate procedure
Fat grafting is not simply:
“adding volume.”
Especially in the face,
even 0.1cc can dramatically change the appearance.
Factors such as:
- skin thickness
- bone structure
- retaining ligaments
- circulation
- facial muscle movement
must all be considered.
In other words,
facial fat grafting is an extremely delicate procedure.
