Many people believe that once fat has been transferred to the breasts, it will simply remain there permanently.

In reality, the long-term survival of transferred fat depends not only on surgical technique, but also on how you care for yourself after surgery.

Transferred Fat Does Not Survive Immediately

Fat grafting involves transplanting your own fat cells into the breasts.

Immediately after surgery, these fat cells have not yet established their own blood supply.

To survive permanently, they must receive oxygen and nutrients through the formation of new blood vessels, a process known as angiogenesis.

Although this process begins soon after surgery, it generally takes up to three months for the transplanted fat to become fully integrated into the surrounding tissue.

This is why we often describe the first three months as the period when your new breast tissue is “growing.”

Compression Can Reduce Fat Graft Survival

Healthy blood flow is essential for newly transferred fat cells.

Excessive pressure on the breasts may reduce blood circulation during this critical healing period.

For this reason, during the early postoperative phase we recommend avoiding:

• Tight underwire bras
• Compression bras that place excessive pressure on the breasts
• Frequent squeezing or firm massage of the breasts

Internal pressure is also important.

Injecting an excessive amount of fat into a limited space can increase tissue pressure, reducing blood flow to the transplanted fat and increasing the risk of fat resorption or fat necrosis.

We Recommend Wireless Bras for the First 1–1.5 Months

Following fat grafting or Hybrid Breast Augmentation, we generally recommend wearing a wireless bra for approximately 4–6 weeks.

Wireless bras provide gentle support while minimizing localized pressure on the newly grafted tissue.

The appropriate type of bra and the duration of use may vary depending on each patient’s recovery, and we provide individualized guidance during follow-up visits.

This patient underwent:

• Revision arm liposuction
• Revision thigh liposuction
• Hybrid Breast Augmentation

She had previously undergone:

• Fat graft breast augmentation
• Motiva Mini 200 cc silicone implants placed in the subglandular plane

For this procedure, additional fat grafting was performed:

• Right breast: 100 mL
• Left breast: 80 mL
