Motiva Demi 250cc Hybrid Breast Augmentation: Profile Choice for Slim Patients2026.10.19

Many patients want more roundness in their breasts but worry that going too large will look unnatural. A silicone implant alone can leave the edge palpable, while fat grafting alone has a per-session ceiling on volume. The answer is Motiva Demi hybrid breast augmentation, which pairs an implant with autologous fat. In this article, based on a slim patient in her twenties who underwent simultaneous full-circumference thigh liposuction, we explain from the surgeon’s perspective why we chose the wide-base, low-projection Demi profile and how the harvested thigh fat was used.

Key Points of This Article

・Motiva profiles are classified by base width and projection; Demi is wide and modestly projecting, suiting slim patients with narrow chests who prefer a natural look

・In Motiva Demi hybrid breast augmentation, the implant provides the base volume while autologous fat smooths the edge and the decolletage slope

・Condense-rich processing increases fat purity, and a subcutaneous fat layer over the decolletage makes the implant edge harder to palpate

・At one month, swelling, edema, and firmness remain; the final result is evaluated at three to six months

・Implant size, injected fat volume, and liposuction area are determined by body habitus and skin elasticity, with indication judged in consultation

Medical Rationale for Choosing the Motiva Demi Profile

The Motiva series is a line of ergonomic silicone implants, and the profile (height in cross section) markedly changes the impression. A flat implant in a slim patient with a narrow chest gives too little projection and looks underwhelming, while a full profile creates a steep slope from the subclavicular region to the breast and announces itself too strongly through clothing.

Demi sits between these options, with a generous base width and a modest projection. The wide base contacts the chest wall broadly, so the slope viewed from the side becomes gentle and the breasts read as naturally present even under clothing. In this case, we measured chest width, breast base width (BW), and skin stretch before selecting Motiva Demi 250cc.

Division of Roles Between Implant and Fat: Foundation and Finish

Hybrid breast augmentation is designed so the implant delivers volume while fat smooths the edge. Trying to achieve a large size with the implant alone makes the edge visible in slim patients with thin skin and creates a step at the upper decolletage. Fat grafting alone, meanwhile, is realistically limited to around 100mL per breast per session, which may fall short of the target volume.

So most of the volume comes from the implant, and a thin fat layer over the upper decolletage and lateral breast hides the edge. In this case, Motiva Demi 250cc provided the foundation and thigh-derived condense-rich fat provided the finish.

Why We Chose Full-Circumference Thigh Liposuction as the Donor Site

Hybrid breast augmentation can draw fat from the abdomen, flanks, thighs, or upper arms. We chose full-circumference thighs here because, even in slim patients, the thighs tend to hold a usable amount of fat and allow a safe harvest of meaningful volume. The supine position also lets us access the full circumference through an anterior inguinal incision, letting us move directly into the augmentation under the same positioning.

Total harvest was 1,000mL, approached as a thin and uniform pass across the anterior, lateral, medial, and posterior thigh through small inguinal incisions. Uniform harvest is the fundamental principle that prevents a depression or residual asymmetry at the donor site.

Making Condense-Rich Fat Through Centrifugation

Harvested fat is not injected as it comes out; anesthetic fluid, blood components, and disrupted adipocytes are separated and removed. In this case, 400mL of pure fat was centrifuged at low speed for a short time, yielding 226mL of concentrated condense-rich fat. Spin speed matters: too fast damages live cells, too slow leaves the fraction poorly separated. Setting a condition that preserves the balance of adipocytes and stem cells is key.

The portion of harvested fat usable for injection varies with the patient and typically falls between 40% and 60%. Estimating this yield correctly is essential to planning the final volume.

Treatment Content, Cost, and Risks of This Case

Treatment content: Motiva Demi hybrid breast augmentation in a woman in her twenties – bilateral Motiva Demi 250cc silicone implants plus injection of condense-rich fat harvested from full-circumference thigh liposuction, 116mL subcutaneously on the right and 110mL on the left. Incisions included an axillary incision for implant placement, inguinal incisions for liposuction, and small incisions below the nipple and anterior to the axilla for fat injection. The procedure was performed under general anesthesia, with an ultrasound device (LSSA, Elsa) used as needed.

Cost: Please refer to the price page for details. The final total is confirmed at consultation.

Main risks and side effects: bruising, swelling, edema, firmness (fibrosis), asymmetry, capsular contracture, infection, implant malposition, calcification or nodules from partial fat necrosis, donor-site depression, and transient sensory change. Results vary between individuals and the photos are one example.

Why We Place Fat in the Subcutaneous Decolletage

In hybrid breast augmentation, fat can be placed subglandularly, intramuscularly, or subcutaneously. In this case, the implant went subglandularly and the fat went subcutaneously. A subcutaneous fat layer softens the step at the implant edge, particularly over the upper decolletage, and smooths the slope viewed from the side.

Slim patients have thin subcutaneous tissue, so the implant edge tends to stand out when used alone. Building a cushion-like fat layer in the subcutaneous plane brings the result closer to “soft to touch” and “the edge does not show through clothing.” Injection is performed in small aliquots spread from multiple directions in thin layers, avoiding a bolus in one spot – bolus injection is a cause of fat necrosis and nodules.

Axillary Incision and Subglandular Pocket

An implant can be placed through a periareolar, inframammary, or axillary incision. We chose the axillary approach so the scar hides along a natural crease and is less visible in daily life. It also keeps the breast itself free of incisions, minimizing any impact on future breastfeeding.

For the pocket, we chose subglandular (between the breast gland and the pectoralis major). Compared with subpectoral placement, the subglandular pocket does not divide muscle, so post-operative pain is relatively less and shape change with muscle contraction (animation deformity) is less likely. This plane suits young women with sufficient glandular thickness. Here, subcutaneous fat injection also adds thickness, compensating for the weak point of subglandular placement – edge visibility.

Pre-operative Measurement and Sizer Fitting

Profile selection cannot be decided by preference alone; objective measurement is essential. Before surgery we measure breast base width (BW), nipple-to-inframammary distance, and skin stretch (pinch test), and overlay these on catalog implant dimensions to narrow candidates. We then place the actual Motiva sizer in a bra and confirm the look in the mirror from frontal, oblique, and lateral views before finalizing the size.

In this case, the Demi 250cc base width fit within the BW and the pinch test showed enough skin elasticity, so the risk of edge protrusion was judged to be low.

Course at One Month and Future Change

At one month, the photo shows forward projection and roundness across the whole breast, with volume added from the decolletage to the chest. This is still an intermediate stage. The capsule around the implant is beginning to form, and the whole breast can feel somewhat firm. The grafted fat is also still choosing between surviving and being absorbed, so the final volume is assessed at three to six months.

The thigh harvest sites also peak in fibrosis-related tightness at one to three months and gradually soften. Course varies between individuals; not everyone follows the same trajectory.

Indications and Limits of Motiva Demi Hybrid Breast Augmentation

Motiva Demi hybrid breast augmentation suits patients who want a certain degree of size increase without an unnaturally emphasized look. On the other hand, the indication deserves careful judgment in the following patients:

– Patients with very low BMI, where harvestable fat may be insufficient

– Patients whose chest width does not match the Demi base width

– Patients with un-investigated nodules in the breast (evaluated with pre-operative ultrasound)

– Patients with elevated bleeding or infection risk from comorbidities or medications

Frequently Asked Questions

Q. How does Motiva Demi differ from Mini?

Both are Motiva profiles, but Demi has a wider base and modest projection while Mini is more compact with a different projection balance. Which suits a patient depends on chest width, skin stretch, and the desired look. We confirm with an actual sizer before deciding.

Q. Can patients in their twenties undergo hybrid breast augmentation?

Age alone does not determine indication. We judge comprehensively based on chest wall and skin, available fat, and overall medical history. Patients in their twenties can be candidates, but timing in relation to future pregnancy or breastfeeding is an important discussion.

Q. Is thigh fat suitable as a donor for breast augmentation?

Thigh fat has relatively uniform cell size and is widely used as a donor for fat grafting. Whether sufficient volume can be harvested depends on body habitus. In patients with very low BMI, we may combine thigh with other sites such as the abdomen or flanks.

Q. Does centrifuged fat take?

Centrifugation removes water and unwanted components to increase fat purity. At an appropriate spin speed, it preserves the balance of adipocytes and stem cells while improving injection efficiency. Graft take also depends on the recipient layer, local blood supply, and injected volume – processing method is not the only factor.

Q. How much of the final result is visible at one month?

At one month, swelling and edema remain and the tissue around the implant is still settling. The overall form becomes visible, but the final evaluation is around three to six months.
