A 24-year-old woman who wished for more breast volume and a natural, rounded shape flowing smoothly from the décolletage. We performed a hybrid breast augmentation combining silicone implants with fat grafting. The photos show her condition 1 month after surgery.

Motiva Mini 250cc implants were placed under the mammary gland through a small incision in the armpit (transaxillary approach). Fat was harvested from the inner and front thighs (260 ml of aspirated fat) and, without centrifugation, 116 ml on the right and 100 ml on the left were injected into the subcutaneous layer of the breasts. The implants provide reliable volume, while the fat softens the upper décolletage and the edges of the implants for a less visible transition.

At 1 month after surgery, swelling and bruising have settled and the projection and roundness of the breasts are clearly visible. The line from the upper chest to the breast flows naturally, with no visible implant outline. Fat retention and skin adaptation will continue to progress, and the texture will become softer over the coming months.

The procedure was performed under intravenous sedation (propofol) combined with nitrous oxide and sevoflurane, using tumescent anesthesia. Operating time was about 1 hour. Incisions were small and located at the front of the armpit, below the nipple, in the armpit, and in the groin. A compression garment (girdle) is worn after surgery.
