Submental Liposuction Profile Design | Thread Lift & Chin Filler Same-Day Combo2026.11.01

Looking at your profile photos, have you felt that the area under the jaw looks heavy, or that the chin blurs into the neck without a clear transition? Massage and exercise rarely change this area, and even slim patients can retain a localized fullness. Submental liposuction is the common solution, but simply reducing the bulge does not always sharpen the profile. In this article, from the viewpoint of our supervising physician, we explain why submental liposuction profile design is defined not by one procedure alone but by a same-day plan that also addresses neck-skin tightening and chin shape.

The short answer

Submental liposuction alone may not fully sharpen the profile. When submental fat, loose neck skin, and chin projection are addressed together in one session, the cervicomental angle becomes more defined and the side view looks cleaner. Individual results vary.

In short, the profile-defining “cervicomental angle” is created by three elements: the amount of submental fat, the degree of neck-skin laxity, and the projection of the chin tip. Treating only one of these often leaves the others visible, while addressing all three in a single session makes it easier to achieve a natural, well-defined profile line.

Submental Liposuction Profile Design Addresses “Fat, Skin, and Chin” in One Session

The profile jawline is determined by three overlapping factors: submental fat volume, laxity of the skin from jaw to neck, and how far the chin tip projects forward. Treating just one of these can leave the untreated factors standing out, blurring the overall line. Submental liposuction profile design combines three actions on the same day — reducing fat, tightening skin, and refining chin shape — so the profile angle (cervicomental angle) becomes more clearly defined.

[Comparison of Submental Area Treatments]

Submental Liposuction Only Liposuction + Thread Lift Liposuction + Thread Lift + Chin Filler
Advantages ・Reduces fat bulge
・Decreases submental thickness
・Reduces fat and supports lax skin
・Tightens jaw-to-neck line
・Addresses fat, skin, and chin shape on the same day
・Sharpens the cervicomental angle
Disadvantages ・Skin laxity may remain
・Change is less visible if the chin is retrognathic
・Chin shape unchanged
・Some tightness characteristic of thread lifts
・More procedures in one session; downtime management needed
・Higher total cost
Best suited for ・Those with clearly abundant submental fat
・Younger patients with preserved skin elasticity
・Those unsure that liposuction alone will tighten enough
・Those bothered by skin laxity
・Those who want a comprehensive profile refinement
・Those also bothered by a retracted chin

Reason 1: Submental Fat Is the Center of the Profile Bulge

Submental fullness arises when subcutaneous fat accumulates beyond the line of the jawbone. Even in slim patients, the shape of the mandible and the way subcutaneous fat is distributed can leave a localized roundness here, flattening the jaw-to-neck angle (cervicomental angle). Submental liposuction uses three small incisions — behind each ear and under the chin — through which a fine cannula is passed along the bony line to reduce thickness evenly. Multidirectional access allows cross-hatched passes, which supports uniform aspiration and helps prevent residual bulges or surface irregularity.

Reason 2: Thread Lifts Support Skin Retraction After Liposuction

When fat is reduced, the overlying skin develops a slight excess. In younger patients this often retracts with the skin’s own elasticity, but neck skin is thinner than facial skin and laxity can remain visible. Placing a pair of bi-needle threads (bidirectional cogged absorbable sutures) from the submental area down into the neck lifts the skin from within and supports post-liposuction skin retraction. The sutures are absorbable; they gradually integrate into surrounding tissue over months while stimulating collagen production.

Reason 3: Chin Projection Defines the Line of the Cervicomental Angle

The impression of the profile is strongly shaped by how far the chin tip projects. Even after submental fat is reduced, a slightly retracted chin can leave the jaw-to-neck boundary indistinct and the chin looking short. A small amount of hyaluronic-acid filler placed along the bony line projects the chin tip forward, which highlights the angle created by submental liposuction profile design. Because hyaluronic acid is gradually absorbed, we deliberately keep the volume and depth conservative.

Our Case

A Case with a Refined Cervicomental Angle (Female, 20s)

A woman in her twenties consulted us about fullness beneath the jaw and a desire to clean up her profile jawline. The photographs compare the side view before surgery with the view two months after a same-day combination of submental liposuction, two neck bi-needle thread lifts, and chin hyaluronic-acid injection (1 cc).

症例写真

Preoperatively, the side view showed roundness under the jaw, a gentle transition from chin tip to neck, and an overall blurred jawline. Her body type was slim, suggesting localized submental fat. Skin elasticity was preserved, but we considered that liposuction alone — because it would not change chin shape — might not create a sufficiently defined cervicomental angle.

症例写真

The plan was a same-day, three-direction design: reduce submental thickness with liposuction, support the skin with neck thread lifts, and project the chin tip with 1 cc of hyaluronic-acid filler. Only 30 ml of fat was aspirated — a small volume — but the submental area is one where even small changes clearly alter the profile.

症例写真

The procedure began with liposuction through three small incisions (behind each ear and under the chin). A fine cannula was passed along the bony line to reduce thickness evenly. Because the face has thin skin and is prone to surface irregularity, care was taken not to over-aspirate; a thin layer of subcutaneous fat was intentionally preserved.

症例写真

Next, two bi-needle threads were inserted from the submental area down into the neck to tighten the skin. Bi-needle threads are bidirectional cogged absorbable sutures used as a type of thread lift. They support the skin excess generated by submental liposuction and give a firmer sense of tightening than thread lifting alone.

症例写真

Hyaluronic acid (1 cc) was then injected in a deep layer close to the periosteum of the chin. Supplementing along the bony line of the chin tip creates a natural flow from the lower lip to the chin tip to the neck, helping the profile “S-curve” emerge. Keeping the volume modest is deliberate — the aim is to supplement the contour without creating any unnatural appearance.

症例写真

Anesthesia used intravenous sedation together with nitrous oxide, and tumescent solution was infiltrated into the submental area before aspiration. Tumescent fluid reduces pain and bleeding while making fat easier to aspirate. After the procedure, a face band was applied for firm compression to limit early-phase swelling.

症例写真

At two months postop, submental thickness has decreased, the angle from the chin tip to the neck has become clearer, and swelling and bruising have largely settled. Firmness of the aspirated area (fibrotic contraction) peaks around one month postop and gradually softens from this stage onward.

症例写真

Comparing the profiles, in addition to reduced submental bulk, the chin outline sits slightly more forward and the border with the neck is more distinct. Skin tightening from the thread lifts continues to settle gradually over 3 to 6 months postop.

症例写真

Because hyaluronic acid is gradually absorbed, the chin shape will continue to change subtly over time. Individual results vary, and the duration of downtime also differs between patients.

Can Submental Liposuction Alone Refine the Profile?

Results Depend on Skin Elasticity and Chin Shape

Submental liposuction alone can create clear improvement for patients with abundant fat and preserved skin elasticity, especially younger patients. However, for patients with loose skin or a slightly retracted chin, reducing fat may still leave a blurred jawline. At consultation, we examine which of the three elements — fat, skin, or bony framework — is driving the profile shape, and plan the necessary combination from there.

Can Bi-Needle Threads and Hyaluronic Acid Be Done on the Same Day?

Same-Day Combination Is Feasible, but Layered Planning and Aftercare Matter

Submental liposuction, neck bi-needle threads, and chin hyaluronic-acid injection are all procedures on adjacent facial regions, and they can be combined in a single session by separating the layers and routes in which they act. Thread lifts work in the mid-to-deep subcutaneous layer, hyaluronic acid is placed in the deep layer near the periosteum, and liposuction operates on the subcutaneous fat layer. Combining them allows anesthesia and downtime to be consolidated and the overall result to be assessed at once. On the other hand, bruising and swelling from all three procedures overlap, so compression and postoperative care become more important.

How Much Downtime and What Risks?

Pain Peaks on Days 2–3, Swelling Settles over 2–3 Weeks

In our clinical experience, pain peaks on postoperative days 2–3 and settles by around day 10, while swelling and bruising resolve over 2–3 weeks. Because the submental area is highly visible, many patients cover it with a mask or face band. Showers are permitted from the next day; bathtubs are permitted from two weeks postop. Fibrotic contraction peaks around one month postop and typically settles over about three months. Possible risks include bruising, swelling, fibrotic contraction, asymmetry, a sensation of skin tightness, shape change as filler is absorbed, infection, and temporary numbness. Because the marginal mandibular branch of the facial nerve runs in the deep tissues of this area, aspirating only in a safe plane is essential. How Long Does the Effect Last?

Liposuction Is Long-Lasting; Threads and Filler Fade Gradually

Because fat cells removed by liposuction do not readily regenerate, the reduction of submental fullness tends to be long-lasting. If overall body weight increases substantially, however, the remaining fat cells may enlarge and some thickness may return. Thread lifts are absorbed into surrounding tissue over months, and the tightening effect from collagen stimulation gradually fades over roughly 6 months to 1 year. Hyaluronic acid is absorbed over months to a year, so maintenance injections may be considered to preserve shape. Submental liposuction profile design is the idea of combining a long-lasting change (fat) with adjuncts that change gradually (threads and filler).

Think Beyond the Submental Area and Design the Entire Profile

The profile jawline is determined by three overlapping factors: submental fat, neck skin, and chin shape. Treating only one of them often has limited impact, while a same-day plan that acts on all three can make the cervicomental angle emerge clearly and the profile look cleaner. Submental liposuction profile design is not about aspirating more, but about identifying the needed elements and combining them.

Which procedures should be combined, and to what extent, depends on facial features, skin condition, and personal preference. Please discuss your condition and the most suitable approach at consultation. Related content is available in our liposuction column list.

Frequently Asked Questions

Can I have submental liposuction alone?

Yes. Patients with clearly abundant submental fat and preserved skin elasticity — particularly younger patients — often achieve a visible profile change with submental liposuction alone. At consultation we also examine skin laxity and chin shape to confirm whether liposuction alone is truly sufficient.

Do thread-lift sutures stay in the body?

Most thread lifts, including bi-needle threads, use absorbable sutures that gradually integrate into surrounding tissue over months. The sutures themselves do not remain; the tightening effect comes from collagen stimulation that persists after absorption.

How long does chin hyaluronic-acid filler last?

It varies with the product, the injection layer, and individual factors, but filler placed in the deep chin layer is generally absorbed over roughly 6 months to 1 year. If you wish to maintain shape, a top-up injection can be considered as absorption progresses.

When can I return to work after submental liposuction?

Swelling and bruising vary between individuals, but because the submental area is covered by a mask, most desk-based workers return within several days to about a week. For patients in client-facing roles, we adjust based on face-band wearing time and the resolution of swelling.

Is there a risk of damaging the facial nerve with submental liposuction?

The marginal mandibular branch of the facial nerve runs in the deep tissues of the submental area, so staying in a safe aspiration plane is critical. We follow the bony line with the cannula and avoid forcing it into deeper layers in order to minimize the risk to the nerves that move the corners of the mouth.
