8 Months After Surgery and 4 Months After Profound Matrix RF

This case involved a comprehensive combination of circumferential upper-arm liposuction, forearm liposuction, surgical arm lift, thread lifting, and Profound Matrix RF treatment.

The patient is now 8 months post-operative from the liposuction and lifting procedures and 4 months post-treatment with Profound Matrix RF.

By combining fat reduction, removal of excess skin, tissue lifting, and radiofrequency-based skin tightening, we aimed to create a slimmer, firmer, and more refined arm contour.

Postoperative Progress

At 8 months after surgery, the overall arm contour has become significantly tighter and slimmer. The arms appear approximately two sizes smaller, with a clearer transition from the upper arms to the forearms.

The patient also continued to lose weight after liposuction, which contributed positively to the final result.

Liposuction reduces the number of fat cells in the treated area, but postoperative weight management remains important. Maintaining a stable or reduced body weight can help preserve and further enhance the improved contour.

At AVAN TOKYO, we value not only the surgical result itself but also long-term body-weight management and postoperative care.

Key Points of This Case

The defining feature of this case is that we treated both excess fat and loose skin.

When significant skin laxity is present, liposuction alone may not produce an adequately firm result. For this patient, we therefore combined several approaches:

This layered approach allowed us to address volume, skin redundancy, tissue position, and skin quality separately.

Why Liposuction Alone May Not Be Enough

Upper-arm fullness is not always caused by fat alone.

The final appearance of the arms is influenced by:

If a substantial amount of loose skin remains after fat removal, the arms may still appear saggy even when the fat volume has been reduced.

For this reason, treatment planning must distinguish between excess fat and excess skin.

About the Surgical Arm-Lift Scar

A surgical arm lift can provide a powerful improvement because it directly removes redundant skin. However, this benefit comes with an unavoidable trade-off: a visible surgical scar.

In general, the scar from an arm lift tends to be more noticeable than the incision used for silicone breast augmentation because the arm-lift incision is usually longer and located along a more visible part of the body.

Scar appearance varies depending on:

Redness, firmness, and pigmentation often improve gradually as the scar matures, but the scar will not disappear completely.

We will continue to monitor this patient over the long term and recommend scar care or additional treatment when appropriate.

The Role of Profound Matrix RF

Profound Matrix RF is a radiofrequency-based treatment designed to deliver controlled thermal energy into the deeper layers of the skin.

In this case, it was used after surgery to address the finer surface concerns that cannot be fully corrected by skin excision or thread lifting alone, including:

The treatment supports gradual tissue remodeling over time. Results are not immediate and may continue to develop over several months.

Profound Matrix RF does not replace surgical skin removal when there is a large amount of excess skin. Instead, it can complement surgery by improving skin quality and residual looseness.

Who May Be a Candidate for This Combination Treatment?

This approach may be considered for patients who:

The most appropriate combination of treatments depends on the patient’s anatomy, skin condition, goals, and tolerance for scarring and downtime.

Risks and Potential Complications

Potential risks include:

All medical and surgical procedures involve risks, and results vary between patients.

Physician’s Commentary

This case demonstrates why effective arm contouring requires more than simply removing fat.

The appearance of the upper arms is determined by the relationship between fat volume, skin excess, tissue support, and skin quality.

For this patient, circumferential upper-arm and forearm liposuction reduced overall volume, while the surgical arm lift and thread lift addressed loose tissue. Profound Matrix RF was then used to improve finer wrinkling and residual skin laxity.

At AVAN TOKYO GINZA LIPOSUCTION CLINIC, our goal is not simply to make the arms smaller. We aim to create a natural and balanced contour based on each patient’s anatomy.

We also explain the limitations and trade-offs of treatment honestly. In particular, patients considering an arm lift must understand that a stronger tightening effect is achieved in exchange for a longer and potentially more visible scar.

Careful patient selection, individualized treatment planning, postoperative weight management, and long-term follow-up are all essential for achieving the best possible result.
