Refining the Upper Arms and Shoulders While Creating a Natural Breast Contour

This case shows the result at 1 month after hybrid breast augmentation combining upper arm and shoulder liposuction with a silicone implant and autologous fat grafting.

The implant used was PERLE MR 255cc.

It was inserted through an axillary incision and placed in the subglandular plane.

Fat was also harvested from the upper arms and shoulders, processed, and then injected into both breasts.

Treatment Design

A total of 300ml of pure fat was harvested from the upper arms and shoulders.

After processing, 180ml of fat was available for grafting.

Appearance at 1 Month After Surgery

The current result is at 1 month postoperative.

At this stage, breast augmentation should not yet be considered complete.

Some tightness may still remain, and the implants may appear slightly higher than their final position. The areas treated with fat grafting also continue to change as postoperative swelling decreases and part of the injected fat is naturally absorbed.

Over the following months, the implants gradually settle and adapt to the surrounding tissues, while the breast contour and softness become more natural.

At AVAN TOKYO, we do not assess the final result based only on the 1-month appearance. We place importance on long-term changes at 3 months, 6 months, and beyond.

Why Combine an Implant with Fat Grafting?

Hybrid breast augmentation combines the advantages of silicone implants and autologous fat grafting.

The implant provides the foundation for breast volume and projection, while fat grafting helps soften the contours and transitions around the implant.

In patients with a slim body type and limited subcutaneous fat, the outline or edge of an implant may be more visible.

Adding fat to selected areas can increase soft-tissue coverage and create a smoother transition through the upper breast and décolleté.

Especially in slim patients, breast augmentation is not simply about deciding how many cubic centimeters of implant to use.

It is also important to consider how the implant will look within the patient’s overall body proportions.

Why Perform Upper Arm and Shoulder Liposuction at the Same Time?

In this case, fat was harvested for breast fat grafting while simultaneously refining the contour from the upper arms to the shoulders.

The appearance of the breasts is influenced by more than the breasts themselves.

Shoulder contour, upper arm volume, the axillary area, décolleté, chest width, and breast volume all affect the overall appearance of the upper body.

By slimming the upper arms and shoulders while adding three-dimensional volume to the breasts, it is possible to create a visual contrast between:

PERLE MR 255cc in the Subglandular Plane

The implant selected for this case was PERLE MR 255cc.

Implant type, volume, and placement are not determined solely by the desired cup size.

We assess chest width, existing breast tissue, soft-tissue thickness, skin elasticity, desired projection, and overall body proportions before selecting the implant.

In this case, the implant was placed in the subglandular plane, and fat grafting was added around the implant to refine the overall breast contour.

Hybrid Breast Augmentation Is Not Complete Immediately After Surgery

At 1 month after surgery, some breast tightness and a relatively high implant position may still remain.

Over approximately 3 months, swelling and tissue firmness gradually decrease.

From around 6 months onward, the implant and surrounding tissues may continue to soften and settle, creating a more natural appearance.

Because some of the injected fat is naturally absorbed, the final volume is evaluated over the longer term.

Rather than focusing only on the initial breast size, it is important to design the procedure while considering how the breast contour will develop several months after surgery and beyond.
