This is a case of a 25-year-old female patient, 1 month after undergoing Secondary Hybrid Breast Augmentation.
The patient had previously undergone silicone implant breast augmentation at another clinic and was satisfied with her breast size. However, she wished to achieve greater fullness in the upper breast (décolletage), improve the appearance of her cleavage, and create a softer, more natural overall feel. Therefore, she underwent fat grafting breast enhancement at AVAN TOKYO.
Secondary Hybrid Breast Augmentation is a procedure that utilizes the existing silicone implants as a foundation while refining the breast shape, texture, and natural appearance through autologous fat transfer. It is particularly effective for improving upper pole fullness and creating a smoother, more natural transition in the cleavage area, which can be difficult to achieve with implants alone.
For this procedure, fat harvested through circumferential arm liposuction was used. Approximately 400 cc of purified fat was obtained, and 140 cc was injected into the right breast and 120 cc into the left breast.
Compared with conventional fat transfer breast augmentation, the injected volume was relatively modest. However, because sufficient volume had already been achieved with implants, this amount was adequate for contour refinement and aesthetic enhancement.
Particular attention was paid to improving the décolletage, cleavage, and midline chest area, which are generally more challenging areas for fat graft survival. The patient has also actively supported fat retention through a high-protein diet, consumption of soy milk and soy-based products, and continued smoking cessation.
At 1 month post-operatively, excellent fullness has been maintained in the upper breast and cleavage areas, resulting in a natural and attractive upper breast contour.
Secondary Hybrid Breast Augmentation combines the volume provided by implants with the softness, shape refinement, and natural appearance achieved through fat grafting, allowing for a more balanced and aesthetically pleasing outcome.
