Breast Reduction & Lift with Replacement of Implants from Another Clinic in a 26-Year-Old Woman | Motiva 210cc Subglandular + Flank Liposuction, 2 Months After Surgery2026.09.05

This case shows a 26-year-old woman who underwent breast reduction and breast lift together with replacement of silicone implants placed at another clinic, plus flank liposuction, photographed from the front 2 months after surgery. The operating surgeon was Dr. Avelino Fujimoto. After silicone implant augmentation elsewhere, she was troubled by heavy, sagging breasts and low nipple position, and chose to have her breasts reduced and lifted while exchanging the implants for Motiva Demi 210cc.
The resection was designed with a Wise pattern (inverted-T). The point where the inframammary fold projects onto the breast meridian was set as the new nipple position, and a 40 mm areola was designed. The nipple-areola complex was elevated on a full-thickness superomedial pedicle and moved upward while preserving its blood supply.
The implants from the other clinic, located in the subglandular plane, were removed, and the capsule was excised on both the anterior and posterior surfaces. The caudal edge of the resected glandular tissue was fixed to the inframammary fold and the pectoralis fascia to recreate the pocket, and new silicone implants (Motiva Demi 210cc) were placed subglandularly. Incisions were made along the inframammary fold, below the nipple, and around the areola, and a drain was placed laterally. For the flanks, tumescent solution was infiltrated and 220 ml of fat was aspirated to refine the waistline.
In the frontal photos, before surgery the breasts were large and ptotic with low-positioned nipples and an ill-defined contour. At 2 months after surgery the breast size is more balanced, the nipple-areola complexes have moved upward, and the breasts have a rounder shape. The waist indentation from flank liposuction is also visible. The nipples in the photos are covered with protective tape.
At 2 months post-op, swelling has largely subsided, but redness and firmness of the scars still remain. The scars will gradually become less noticeable over the next 6 to 12 months, and the implants will continue to settle and the shape will keep changing. Posture, distance, lighting, and weight changes at the time of photography also affect how the results appear. The incision design, pedicle choice, implant volume, and implant plane are selected based on glandular volume, degree of ptosis, skin elasticity, chest wall shape, and surgical history.
