5-Month Results: 360° Arm, Thigh & Calf Liposuction with Hybrid Breast Augmentation | 27-Year-Old Patient
A More Refined Body Contour: Combining Full-Body Liposuction with Hybrid Breast Augmentation in Tokyo, Japan
At AVAN TOKYO Ginza Liposuction Clinic, our approach to body contouring focuses on more than simply removing fat or increasing breast size. We aim to create elegant, balanced proportions by considering the relationship between different areas of the body.
This case features a 27-year-old woman who underwent 360° arm liposuction, accessory breast (axillary fat) removal, circumferential thigh liposuction, above-the-knee liposuction, calf liposuction, and hybrid breast augmentation.
At five months postoperatively, her slimmer arm contours create a beautiful contrast with her enhanced breast volume, contributing to a more defined and harmonious overall silhouette.
By combining liposuction with breast augmentation, we can address multiple aspects of body proportion within a carefully planned surgical approach.

Three Key Elements of This Body Contouring Procedure
1. Creating Beautiful Proportions Through Arm Liposuction and Breast Enhancement
One of the most important aspects of this procedure was achieving a balanced relationship between the arms and breasts.
When designing upper arm liposuction, our goal is not simply to remove as much fat as possible. Instead, we carefully evaluate the natural anatomy of the shoulders, arms, and axillary regions to create a slimmer, smoother contour.
In this case, circumferential arm liposuction and accessory breast fat removal were performed to refine the upper body silhouette.
By reducing excess fat around the arms and underarm areas, the enhanced breast volume becomes more visually prominent, creating a more feminine and defined body contour.
This approach is particularly relevant for patients who want to improve their overall proportions rather than focus on breast size alone.
2. Hybrid Breast Augmentation Using PERLE MR 280cc Implants and Fat Grafting
Hybrid breast augmentation combines the structural volume of silicone implants with the contouring benefits of autologous fat grafting.
For this patient, PERLE MR 280cc silicone implants were placed in the subglandular plane.
An additional 80cc of purified fat was injected into each breast, primarily in the subcutaneous tissue, to provide additional soft-tissue coverage around the implants.
This technique aims to:
● Enhance breast volume while maintaining natural-looking contours.
● Soften the visible transition between the implants and surrounding tissue.
● Reduce the appearance of implant edges in areas with limited soft-tissue coverage.
● Improve overall breast shape and proportion.
Fat grafting can be particularly useful when additional tissue coverage is desirable. However, fat retention varies between individuals, and not all injected fat will survive permanently.
Our surgical planning considers each patient’s existing breast tissue, skin thickness, chest anatomy, and desired breast appearance.
3. Comprehensive Lower-Body Contouring: Thighs, Knees, and Calves
In addition to upper-body contouring, this patient underwent circumferential thigh, above-the-knee, and calf liposuction.
The goal was to create a smoother transition from the thighs to the knees and calves while maintaining natural anatomical proportions.
Liposuction planning must account for several important factors, including:
● Subcutaneous fat thickness and distribution.
● Underlying skeletal structure.
● Muscle volume and shape.
● Skin elasticity and the risk of postoperative irregularities.
At AVAN TOKYO, we believe that preserving the natural transitions between anatomical regions is essential for achieving aesthetically balanced results.
Excessive fat removal can lead to contour irregularities, loose skin, or unnatural depressions. Therefore, each treatment area requires careful evaluation and an individualized surgical plan.
Postoperative Recovery: Results at 1 Week, 1 Month, 4 Months, and 5 Months
Recovery following liposuction and hybrid breast augmentation is a gradual process.
1 Week Postoperative
Swelling, bruising, tenderness, and temporary changes in body contour are common during the early recovery period.
1 Month Postoperative
Initial swelling generally begins to decrease. However, firmness, residual swelling, and tissue contracture may still be noticeable in liposuction areas.
4 Months Postoperative
As postoperative swelling and tissue firmness gradually improve, the contours of the arms, legs, and breasts become easier to evaluate.
5 Months Postoperative
At five months, this patient’s postoperative photographs demonstrate a more defined upper-body silhouette, with slimmer arms creating a noticeable contrast against the enhanced breasts.
Her overall proportions also reflect the combined approach to upper- and lower-body contouring.
Although substantial changes may be visible by this stage, the healing process is not necessarily complete. Breast softness, implant settling, swelling, and tissue remodeling may continue to evolve over six months to one year or longer.
Final results and recovery timelines vary between patients.
Dr. Shin Moriwaki’s Perspective: Beauty Is About Overall Proportion
“When I perform liposuction and breast augmentation together, I focus on the balance of the entire body rather than treating each area independently.
Even with the same breast implant size, the overall appearance can be significantly influenced by the contours of the upper arms, shoulders, and surrounding areas.
By creating slimmer arms while adding appropriate volume to the breasts, we aim to achieve a more elegant and balanced silhouette.
For patients undergoing extensive liposuction involving the thighs, knees, or calves, it is equally important to understand the anatomy of each region and avoid excessive fat removal.
At AVAN TOKYO, our approach is to develop individualized surgical plans based on each patient’s anatomy, aesthetic goals, and safety considerations.”
