Second Breast Fat Grafting at Age 27 | 200ml per Side, 3 Months2026.09.07
This is a case of a second fat grafting breast augmentation performed on a 27-year-old woman, shown three months after surgery. Following her earlier fat grafting procedure, the aim this time was to add further volume and roundness to the bust while also refining the contour of the abdomen, so liposuction of the whole abdomen and grafting of the harvested fat were carried out in the same session.
Fat was harvested from the whole abdomen (upper abdomen, lower abdomen and flanks). After infiltrating the tissue with 1,200 ml of tumescent solution, a laser device (LSSA) was used at power 4 for 3 minutes 33 seconds to soften the fibrous layers, and the fat was aspirated while varying the direction and depth of the cannula. Because securing enough graftable fat can be difficult in a second augmentation, harvesting from the inframammary area was added during surgery, bringing the total aspirate to 1,150 ml.
Of the harvested fat, 700 ml of pure fat was centrifuged (600 rpm for 1 minute) to remove oil, fluid and anaesthetic solution, leaving 430 ml prepared for grafting. On the right side, 60 ml was placed under the mammary gland, 60 ml within the pectoralis muscle and 80 ml subcutaneously; on the left side, 60 ml under the gland, 60 ml within the muscle, 20 ml under the muscle and 60 ml subcutaneously, for a total of 200 ml per side. Distributing the graft across layers rather than in a single pocket helps limit the risk of lumps while building roundness from the base.
The operation took about one hour and twenty minutes. Small incisions were made below the nipples, at the anterior axilla, the navel and the groin, and a compression bodysuit was worn afterwards. Harvested areas tend to feel firm and tight for roughly one to three months, and grafted fat also needs several months for swelling to settle and for the take to stabilise. At three months after surgery, the transition from the décolleté to the breast, and the height and roundness seen from an oblique angle, have changed.
In a second fat grafting breast augmentation, the plan depends on how the previous graft took and on how much fat can be harvested. Combining the procedure with liposuction of an area of concern allows the waistline and breast volume to be addressed at the same time; on the other hand, the take rate of grafted fat varies from person to person, and risks such as lumps, calcification and contour irregularity at the donor site exist. Results and recovery vary between individuals, so if you notice anything of concern, please consult your surgeon rather than judging on your own. The suitability of the procedure and the surgical technique should be confirmed through examination and consultation.
