Hybrid Breast Augmentation (Motiva Demi 270cc + Fat Grafting) with Full Upper Arm, Full Thigh and Upper Back Liposuction | 26-Year-Old Woman, 3 Months After Surgery2026.09.19
A 26-year-old woman who wished for more breast volume and slimmer lines on her upper arms, thighs and back. On the same day, we performed a hybrid breast augmentation combining silicone implants with fat grafting, together with liposuction of the full upper arms, full thighs, and the area above and below the shoulder blades. The photos show her condition 3 months after surgery, seen from behind.
Liposuction was performed on three areas—the full upper arms (back, shoulder and front), the full thighs, and the upper back around the shoulder blades—using the ultrasound-assisted liposuction device Elsa. The aspirated fat volume was 1,750 ml from the thighs, 800 ml from the back, and 400 ml from the upper arms. Motiva Demi 270cc implants were placed under the mammary gland through a small incision in the armpit (transaxillary approach). From the harvested fat, 400 ml of pure fat was centrifuged and concentrated to 250 ml, and 120 ml on the right and 100 ml on the left were injected into the subcutaneous layer of the breasts.
At 3 months after surgery, the thighs are narrower on both the inner and outer sides, with a visible gap between them. The line from below the buttocks to the back of the thighs is cleaner, and the thickness from the back to the upper arms has decreased, giving the whole rear view a tighter appearance. Swelling and bruising have settled, and the skin will continue to adapt over the coming months.
The procedure was performed under intravenous sedation (propofol) combined with nitrous oxide and sevoflurane, using tumescent anesthesia. Operating time was about 4 hours. Incisions were small and located below the buttocks, in the groin, at the elbows, at the front and back of the armpits, below the nipples, and below the shoulder blades. Compression garments (girdle and upper arm supporters) are worn after surgery.
