Upper Arm Liposuction With Thread Lift: Combined Design for Skin Looseness2026.10.21

In your 40s, it’s not only arm thickness that starts to bother you — the looseness of the “wing” area when you raise your arm, and the gradual loss of skin firmness, often become concerns at the same time. This article presents a case of a woman in her 40s who underwent circumferential liposuction of the upper arms along with the axillary flanks and accessory breasts, combined with the insertion of 12 thread lifts in each upper arm. As the supervising physician, I will explain the medical rationale behind choosing an arm liposuction thread lift combination, the insertion design, and the progress at 1.5 years postoperatively.

Key Takeaways

・An arm liposuction thread lift combination is a design that addresses both fat volume and skin looseness in a single session.

・In the 40s, skin firmness gradually declines, so liposuction alone often leaves looseness in the “wing” area.

・By continuously suctioning the full circumference of the upper arm along with the axillary flanks and accessory breast, the line from shoulder to armpit to arm appears connected.

・Thread lifts use absorbable cogged sutures placed under the skin to provide a lifting vector.

・Results vary between individuals; the volume suctioned and the number of threads must be designed based on body type and skin condition.

Case Overview: Circumferential Arm Liposuction With 12 Thread Lifts

This case is a woman in her 40s. Her concerns were arm thickness, looseness of the “wing” when raising the arm, and bulkiness around the armpit (axillary flanks and accessory breast). We performed circumferential liposuction of the upper arms (wing, shoulder, anterior), the axillary flanks, and the accessory breasts, and inserted 12 thread lifts in each upper arm. The photographs show the preoperative state and the state at 1.5 years postoperatively, with the arm raised laterally from behind. Preoperatively, there was thickness from the posterior upper arm to the armpit, and the wing area appeared to droop. At 1.5 years postoperatively, the posterior line of the arm is cleaner, the contour from elbow to armpit approaches a straight line, and the whole arm appears slimmer.

Treatment, Cost, and Risks for This Case

Treatment performed: Circumferential upper arm liposuction, axillary flank liposuction, accessory breast liposuction, axillary shaving, and 12 thread lifts inserted into each upper arm. Anesthesia was general anesthesia (intravenous plus inhalation), and an ultrasound device (LSSA, Elsa) was used as needed. Total aspirated fat was 675 ml, and 1,000 ml of tumescent solution was infused.

Cost: Please see our price page.

Main risks and side effects: Bruising, swelling, fibrosis (firmness, tightness), asymmetry, infection, numbness, and scarring. Thread lifts can cause dimpling, tightness, discomfort during absorption, and rarely thread exposure.

Results vary: The photos represent one example. Final appearance and recovery time depend on body type, skin condition, and age.

Why Liposuction Alone Can Leave “Wing” Looseness

The skin of the upper arm is relatively thin, second only to the face and décolleté. When subcutaneous fat is suctioned, the volume previously occupied by fat is reduced, and the overlying skin is left in excess. If the skin’s own elasticity (the tension of collagen and elastin fibers) is strong enough, it will retract and conform to the new contour over time. However, from the 40s onward, dermal fibrous components gradually decline, and skin retraction is not as vigorous as in younger patients. As a result, after liposuction alone, patients may notice “volume is reduced, but the skin ripples when the arm is raised” or “the lower edge of the wing looks loose.” One option to address this residual skin looseness is a same-session thread lift.

Three Medical Reasons for an Arm Liposuction Thread Lift Combination

1. Mechanical support for skin retraction

Thread lifts use absorbable cogged sutures placed in the subcutaneous layer to pull the skin in a defined direction. By applying a physical lifting force to the skin loosened by liposuction, the threads assist retraction during the fibrosis phase. The idea is to gather the “excess skin” that liposuction alone cannot keep up with.

2. Induction of neocollagenesis

As the thread material (absorbable polymers such as PDO, PLLA, PCL) breaks down in the subcutaneous tissue, a mild fibrotic response is triggered, promoting new collagen formation. Even after the threads are absorbed, the subcutaneous support tissue is thickened, restoring skin firmness. This two-stage effect — immediate mechanical support and later collagen reinforcement — is designed to overlap with the skin looseness that follows liposuction.

3. Consolidating downtime in a single session

If liposuction and thread lifts are performed on separate days, patients need anesthesia, compression, and recovery periods twice. Combining them in a single session consolidates downtime. More importantly, the fibrosis phase following liposuction overlaps with the period when thread-lift support is strongest, so skin retraction and the lifting vector work simultaneously. This synergy is easier to achieve with same-day combination than with staged treatments.

Design of the 12 Thread Lifts

In this case, 12 threads were inserted into each upper arm. The number of threads is not simply “more is better” — it is determined by the degree of skin looseness, the length and circumference of the arm, and the balance with the suctioned volume. In this case, we considered two vectors: one to lift the lower edge of the wing area, and one to support the entire skin longitudinally along the arm’s long axis. The threads were placed along the natural tension lines of the skin from axilla to elbow, chosen to minimize tightness or discomfort during arm movement. Inserting too many threads causes them to interfere with each other, risking dimpling and visible steps, so we prioritize the minimum number that provides maximum support.

Progress at 1.5 Years: Synergy of Skin Retraction and Thread Lift

Skin retraction after upper arm liposuction is generally strongest around 3 months postoperatively and gradually settles between 6 months and 1 year. The mechanical support of a thread lift lasts for the duration of thread absorption (6 months to 2 years depending on material), during which neocollagenesis takes over the supportive role. At 1.5 years, the fibrosis and swelling from liposuction have largely settled, and tissue remodeling from the thread lift is nearing completion. In this case, the posterior line of the upper arm became smooth and continuous, and the drooping of the wing when raising the arm was reduced. Timing and extent of change vary between individuals, but the key is to design the “liposuction response” and the “thread-lift response” so that they progress as one continuous process rather than two separate events.

Indications and Limitations of an Arm Liposuction Thread Lift Combination

An arm liposuction thread lift combination is suited to patients who are concerned about both fat volume and skin looseness. Patients with mild to moderate skin laxity — looseness that is not very visible when the arm is at rest but becomes apparent when the arm is raised or extended — are good candidates. However, when skin laxity is severe, with excess skin visible even at rest and tissue hanging down toward the elbow, thread lifts alone cannot provide enough lift. In such cases, surgical skin excision (brachioplasty) must be considered. Indication is decided by comprehensively assessing skin thickness and elasticity, fat volume, age, and lifestyle. Compression Garment and Fibrosis Care

After surgery, patients wear a bolero-type compression garment for the upper arms for a defined period. The compression garment reduces swelling and bruising, and helps the skin conform evenly to the newly suctioned space — it is an important part of the medical treatment. When thread lifts are combined, we give detailed instructions on garment size and wearing time to avoid conflict between the thread vectors and the compression. During the fibrosis phase (1-3 months postop), the skin can feel firm. Gradual resumption of lymphatic drainage and light exercise helps it settle. Because tightness may develop around thread insertion sites, we ask patients to avoid forced range-of-motion exercises for 2-3 weeks. For related articles, see our liposuction column index.

Frequently Asked Questions

Q. Who is a good candidate for an arm liposuction thread lift combination?

Patients who are concerned about both arm thickness and the looseness or loss of firmness that appears when the arm is raised are good candidates. Those with mild to moderate skin laxity, who do not yet need an incisional lift, benefit most. When laxity is severe, incisional lifting (brachioplasty) must be considered — this is decided during consultation by examining skin thickness and elasticity directly.

Q. How long does the thread lift effect last?

The threads themselves are absorbed over 6 months to 2 years depending on the material. Changes in subcutaneous tissue from the induced neocollagenesis are thought to persist longer, but age-related skin changes continue, so the duration of noticeable effect varies between individuals. It is not possible to state a fixed number of years.

Q. Is downtime longer than liposuction alone?

Adding a thread lift to liposuction does not usually extend overall downtime significantly. However, tightness or mild discomfort may persist at thread insertion sites for 1-2 weeks, during which forceful backward rotation of the arm should be avoided.

Q. Does more suctioned fat always mean a better result?

No. The volume of fat removed alone does not determine the result. Avoiding over-suctioning of the superficial layer according to skin thickness, and balancing the remaining fat layer, prevents irregularity and sagging. In this case 675 ml was suctioned, but this number is a “result,” not a “goal.”

Q. Can a thread lift alone improve upper arm looseness?

If a large volume of fat remains, the weight pulls against the threads and reduces their effect. In patients with substantial subcutaneous fat, performing liposuction first — or combining it on the same day — allows the threads to provide better support. At consultation, we explain the suitability of liposuction alone, thread lift alone, and the combination for each case.
