How long does the tightness after hybrid breast augmentation last? Does the silicone implant edge really disappear over time? These are among the most frequent questions we hear in consultation. Based on one case of hybrid breast augmentation at 6 months (Motiva Mini 290cc plus liposuction of the upper arms, shoulders and accessory breast, with 70 ml of fat grafted per side) we performed on a slim woman in her early twenties, this article explains what happens inside the breast at this stage and how the supervising surgeon evaluates the result.

Case overview: hybrid augmentation for a slim body

The case is one example of a slim woman in her early twenties. She wanted a natural-looking increase in breast volume and did not want the implant outline to show through the skin. After counseling, she chose hybrid breast augmentation combining a silicone implant with autologous fat grafting.

Key surgical points:

・Implant: Motiva Mini 290cc, inserted through a transaxillary (armpit crease) incision and placed subglandularly

・Donor sites: upper arms, shoulders (which also serve as body-contouring areas) and accessory breast

・Fat grafting: pure fat harvested was concentrated by centrifugation, and 70 ml per side was distributed in the subcutaneous layer with a focus on the décolletage

Slim patients tend to have thin mammary tissue and subcutaneous fat, so an implant used alone can leave the upper edge visible. Hybrid breast augmentation is designed to compensate for this by layering “cover fat” over the implant.

What happens in the tissue at hybrid breast augmentation at 6 months

Both physiologically and clinically, hybrid breast augmentation at 6 months is the stage when the look and feel of the breast come close to their final state. Three representative changes occur at this time.

1. The capsule around the implant matures

Whenever a silicone implant is placed in the body, a thin fibrous membrane (capsule) forms around it as a normal reaction to a foreign body. During the first 1–3 months this capsule is inflammatory and slightly thick, causing the breast to feel tight and firm. Around hybrid breast augmentation at 6 months, inflammation subsides and the capsule matures into a thinner, softer state. The feel changes from a bouncy firmness to a softer, more yielding texture.

2. The volume of grafted fat that has taken becomes stable

Transplanted fat receives new blood vessels from the surrounding recipient bed, and cells that fail to receive circulation are gradually resorbed. Clinically, this resorption is largely complete between 3 and 6 months, after which the volume does not change much. Fat that remains at hybrid breast augmentation at 6 months is generally maintained as the patient’s own tissue in the long term.

3. Cover fat over the décolletage hides the implant edge

In this case, 70 ml per side was distributed into the subcutaneous layer with the décolletage as the focus. The design uses the implant to create base volume and a thin layer of fat on top of it to camouflage the implant edge. At hybrid breast augmentation at 6 months, the upper edge of the implant is difficult to see from the front, and pinching the skin lightly reveals a soft layer of fat.

Treatment, cost and risks for this case

■ Treatment: Hybrid breast augmentation (Motiva Mini 290cc, transaxillary incision, subglandular placement, fat grafting 70 ml per side) plus liposuction of upper arms, shoulders and accessory breast (400 ml of pure fat harvested, concentrated by centrifugation).

■ Cost: JPY 1,073,000 (tax included, monitor price). Hybrid breast augmentation JPY 998,000 (includes one donor-site liposuction of upper arms and shoulders) plus accessory-breast liposuction JPY 75,000 with a set discount. The monitor price requires consent to photo and video documentation and may be discontinued without prior notice. The final total is confirmed at consultation.

■ Main risks and side effects: swelling, bruising, pain, edema, skin irregularity or tightness, asymmetry, pigmentation, small incision scars, and rarely infection, hematoma, or numbness. Liposuction donor sites may show skin laxity, rippling, or a pulling sensation. Fat grafting carries individual variation in graft survival and possible calcification, lumps, or oil cysts, as well as donor-site irregularity. Implants may cause capsular contracture, rupture, malposition, rippling, infection, or sensory changes.

Remaining risks and what we observe at this stage

Most symptoms settle by 6 months, but the following points continue to be monitored.

・Capsular contracture (Baker classification): can progress slowly beyond this stage. If only one side becomes firm or distorted, please seek examination promptly.

・Fat calcification and oil cysts: some fat that fails to take may degenerate; this is visible on mammography or ultrasound at routine screening. Always inform the technician of your breast augmentation history.

・Slight asymmetry: may still be present at 6 months but often continues to improve as the capsule matures.

Q. Can I consider the result nearly complete at hybrid breast augmentation 6 months?

Capsule maturation and fat graft take are largely complete at this stage, so most of the look and feel is close to the final state. Fine contour changes and minor asymmetry may still refine over the next year or so.

Q. My breasts still feel a little tight at 6 months — is that abnormal?

Capsule maturation varies among individuals, and some tightness at 6 months is not unusual. However, if you notice sudden hardening, pain, or worsening asymmetry, please see your surgeon rather than self-assessing, as this can be an early sign of capsular contracture.

Q. How much of the grafted fat remains?

Generally 30–70% of the injected fat is thought to take permanently, with individual variation by tissue quality, region, and injection layer. Fat that remains at hybrid breast augmentation 6 months is expected to persist as the patient’s own tissue thereafter.

Q. Can I return to normal sports and gym workouts at 6 months?

Most patients can resume exercise without restriction at this stage, but activities involving strong contraction of the pectoral muscles or contact sports should be confirmed with your surgeon just in case.

Q. When can additional fat grafting be considered?

After about 6 months, once the capsule and fat take have stabilized, is a suitable time to consider adding more fat. We assess the breast condition and your goals before proposing an ideal timing.
