Scalp Treatment with Stem Cell Conditioned Media: What To Do and Avoid on the Day and the Day After — Dr. Moriwaki on Washing, Exercise, Alcohol, and Styling Product Aftercare Design2026.07.12

At AVAN TOKYO Ginza’s hair regenerative medicine program, we deliver stem cell conditioned media around the scalp hair follicles using fine needles, and also combine it with Morpheus8 as a drug delivery vehicle. Almost every consultation includes practical questions such as “Can I shampoo tonight?”, “Can I go to the gym tomorrow?”, or “I have wine plans this weekend—is that OK?” In reality, whether or not stem cell conditioned media reaches its full potential depends not only on the injection itself but also on the aftercare design over the 24 to 72 hours following the procedure. In this article, Dr. Moriwaki outlines what to do and avoid after scalp treatment, from the perspectives of skin wound healing and growth-factor delivery.

Key Points of This Article

・After scalp treatment with stem cell conditioned media, the guiding principle for the first several hours to one day—while the microwounds close—is “do not touch, do not heavily wet, do not apply heat.”

・Shampooing should generally restart the following morning with lukewarm water. On the evening of the procedure, avoid the bathtub and take only a light shower from the neck down.

・Intense exercise, alcohol, and sauna dramatically increase blood flow and can prolong bruising and redness, so avoid them for 48 hours.

・Perms, hair color, and strong styling products should be postponed until the skin barrier restores itself, roughly 7 to 14 days later.

・For strong pain, pus, widespread redness, or fever over 38°C, contact the clinic instead of self-managing.

What Happens on the Scalp Right After Treatment — Preparing the Ground for Stem Cell Conditioned Media

Immediately after scalp injection or microneedle RF, the skin holds countless invisible microwounds. The outermost barrier (stratum corneum and sebum film) closes over the following several hours to a full day. During this period, the skin is more vulnerable to external microbes and physical stimuli than usual, while a “wound-healing cascade” of inflammatory cytokines and growth factors begins locally.

Growth factors contained in stem cell conditioned media, such as VEGF, IGF-1, and HGF, are thought to act on the follicular microenvironment by riding this healing cascade. In other words, the scalp right after the procedure is both “the stage where the effect is born” and “a delicate window that must be protected.” There is individual variation, but daily practice shows clearly that how a patient spends this early period influences the length of downtime and the impression of the final result.

What To Do and Avoid on the Day of Treatment

Shampoo Only From the Next Morning, as a Rule

Shampooing on the evening of treatment applies surfactants, heat, and mechanical stimulation simultaneously to a scalp whose microwounds have not fully closed, and it is safer to avoid it. Showering from the neck down is fine, but avoid running strong hot water over the scalp. The next morning, use lukewarm water (around 38°C) with a low-irritation shampoo, lather well, and gently wash with the pads of your fingertips.

Do Not Touch or Scratch

Patients with an unconscious scratching or pulling habit need particular care. It can cause bacterial infection and pigmentation. If you feel itchy, at most place a cool towel lightly on the area.

Do Not Apply Anything to the Scalp

Avoid hair-growth tonics, alcohol-containing tonics, and styling products entirely until the next morning. The opened microchannels allow irritant components to enter easily and can prolong inflammation and redness.

What To Do and Avoid From Day 2 to Day 3

Avoid Intense Exercise and Sauna for 48 Hours

Activities that sharply raise blood flow (running, weight training, hot yoga, sauna) can widen bruising and delay resolution of redness. Walking and light stretching are fine.

Postpone Alcohol at Least Until the Next Day

Alcohol dilates peripheral blood vessels and prolongs redness and swelling at the treated area. Even after the next day, staying moderate for another 1 to 2 days is preferred. If you have an important dinner scheduled, it is safer to adjust the timing of the procedure itself.

Prefer Showers Over Baths for the Following Days

Avoid long baths and heat-stone baths for around 3 days after treatment. The scalp pores open and mix with sebum and sweat, creating an environment where bacteria can proliferate.

Avoid Tight Caps and Helmets

If you must wear a hat outdoors, choose a loose, breathable one for a short time only. Tight motorcycle or bicycle helmets that compress the scalp put mechanical load on follicles that are still healing.

What To Watch For During Weeks 1 to 2

Perms, hair color, and bleach should be avoided until the skin barrier fully recovers, generally 7 to 14 days post-treatment. Alkaline agents and oxidants irritate even a healthy scalp and can cause strong inflammation or contact dermatitis on a healing scalp.

Avoid blowing hot dryer air directly onto the scalp for extended periods; keep the dryer 10 to 15 cm away from the hair. Head spas and strong scalp massage should also wait about a week, allowing the injected stem cell conditioned media to integrate with the tissue. For AGA management and general skin-care guidance, the Japanese Dermatological Association‘s guidelines are also useful references.

How the Scalp Changes and When to See the Clinic

From immediately after the procedure through the next day, small punctate redness at the injection sites, mild swelling, and small bruises are natural reactions that typically improve within 3 to 7 days. Some scalp discomfort or itching is common and does not require excessive concern.

However, do not self-manage signs such as pain that keeps intensifying, pus or heavy discharge, widespread redness, or fever over 38°C—please contact the clinic. We also welcome consultations that include your past treatment history and current medications. Frequently Asked Questions

Q. What if I really want to shampoo the same evening as the procedure?

Showering from the neck down is fine, but please wait until the next morning to shampoo the scalp. If you really must remove sebum or styling products, rinse only lightly with lukewarm water and do not rub hard with your fingertips.

Q. I have a dinner tomorrow with wine planned—what should I do?

If possible, skipping alcohol will keep redness and swelling more stable. If declining is impossible, keep it to a small amount of low-alcohol drink and hydrate well. If you know about the plan beforehand, adjusting the treatment date by a few days is safer.

Q. Should aftercare be even stricter when combined with Morpheus8?

Yes. Because microneedle RF adds thermal effects, redness and mild swelling tend to last a few days longer. Extend the pause on exercise, alcohol, and heat exposure (sauna, long baths) by another 1 to 2 days, and fully postpone shampoo until the next morning to stay safe.

Q. Bruising has remained for a few days after treatment. Is that OK?

Punctate to small-area bruising at injection sites typically resolves naturally in 5 to 10 days. Please contact us if the area keeps expanding or is accompanied by strong tenderness on pressure.

Q. What can I do in daily scalp care to maximize the effect?

Sustained attention to four points—sleep; protein and iron-and-zinc-rich diet; gentle non-rubbing shampooing; and UV protection—supports the follicular-environment improvement provided by stem cell conditioned media from the foundation up.
