Early Signs of AGA Visible Under the Microscope

Hair Diameter Variation (Anisotrichosis)

On a healthy scalp most hairs are thick anagen hairs of a fairly uniform caliber. Once AGA begins, DHT (dihydrotestosterone) shortens the anagen phase of the hair cycle, and more and more hairs enter telogen before they have had time to fully thicken. Under magnification, extremely fine vellus-like hairs and thick terminal hairs appear together in the same field. This unevenness in thickness is called anisotrichosis, and when more than 20% of hairs show diameter differences, AGA is strongly suspected. The greatest strength of magnified observation is that it can catch this early stage that a mirror simply cannot.

Reduction in Hairs Per Follicular Unit

Hair naturally grows in “follicular units” of two to three hairs sharing one pore. As AGA progresses, these units gradually shift toward single-hair units, so the proportion of pores producing only one hair increases. Counting units on the vertex and comparing them to the occiput under the microscope lets us objectify both the distribution and the stage of AGA. When the occiput is dominated by two- and three-hair units but the vertex is dominated by single-hair units, this is one of the classic patterns of early AGA.

Peri-follicular Inflammation, Sebum and Keratinization

In early AGA, mild peri-follicular findings — erythema and brownish pigmentation around the follicular openings — can also be observed. Excess sebum, abnormal keratinization, dandruff and clogged pores worsen the scalp environment and can blunt the response to the regenerative approaches discussed below, so they should be assessed before layering treatments. Fixing the scalp environment alone will not stop AGA, but stacking treatments without preparing that environment reduces the return on investment.

Objectify Response with “Magnified Photographs”

After starting treatment, we photograph the same sites under the microscope every three to six months and objectively track diameter variation, hairs per follicular unit, and the thickening of vellus hairs. Relying on “I feel like it increased / I feel like nothing changed” makes it too easy to waver on continue-or-stop decisions.
